Responsibilities
The role of the Intake RN at hope Staffing Plans will be responsible for knowing require outreach activities within the community and the assessment of eligibility and safety of potential members. Collaborate with other members of the Intake/Outreach team and the clinical team to assure the appropriate provision of services to new members. Communicate with potential members, family, referral sources and physicians to assure a seamless transition to managed long term care.
Qualifications
Must be a self-assured professional with a current New York State Registered Nurse License required. Bachelor’s degree in Nursing preferred. One (1) year experience in an acute care setting required. Must have UAS- NY experience. Home care and management experience preferred. MS Office required. Effective oral, written, communication and interpersonal skills required. Bilingual Spanish, Chinese, Creole, Russian or Punjabi Preferred
Compensation: $95,000 – $105,000 Annually
